CHI Health Partners Grand Island chapter, after its origination in 2014, has seen strong growth in both provider membership as well as value-based care offerings. The number of and lives covered by our value-based contracts have seen huge growth, and our recently enhanced data analytic tools position us well going to the future. We have shown success in showing strong patient outcomes and quality while lowering the total cost of care. We have a strong track record with supporting care coordination activities and have had significant shared savings.

CHI Health Partners, Kearney Chapter, has seen amazing growth in value-based care with payers and employers since its founding in 2014. CHP helps providers improve patient care through our data tools and helps providers and patients through care coordination activities. CHP funds the data tools and helps fund care management activities. CHP works to control per capita cost, improve quality metrics, and improve patient and provider experience. CHP has been able to share significant savings with our providers and facilities.

CHP gives providers access to powerful analytical tools that assist in care coordination activities. CHI Health Partners began as a physician-hospital organization named UniNet in 1998, comprised of the hospitals and aligned physicians of Alegent Health and Creighton University Medical Center in Omaha, Nebraska. Since its inception, the network has focused on bringing dedicated providers together to improve the delivery and coordination of care. This organization has been the regional leader in clinical integration and value-based care, forming the state’s first Medicaid ACO in 1999 and one of the first Medicare ACOs in the state in 2013. In 2012 UniNet received funding from CMMI through the Community-based Care Transitions Program to begin building an outpatient care management program to complement our in-patient care management team and coordinate care across the continuum. In 2012 Creighton and Alegent merge their organizations and in 2013 became wholly sponsored by CHI, which included hospitals across the state.
